Our Online Application - Quick, Easy and Convenient!
By completing the online enrollment application you will be submitting an actual enrollment request to join a ConnectiCare VIP Medicare Plan. In most cases, once your application is submitted electronically, you're done. ConnectiCare will process your application and contact you if we need more information. Otherwise you’ll receive your Welcome Kit and Member ID card in the mail.
Medicare beneficiaries who are eligible to join a 2013 ConnectiCare VIP Medicare Plan may do so by selecting the "Enroll in a 2013 Plan" button located below.

ConnectiCare Representatives are standing by to take applications by phone. Medicare beneficiaries who are eligible to join a ConnectiCare VIP Medicare Plan can call us toll-free at 1-877-224-8221, Monday through Friday from 8 a.m. – 6 p.m. (TTY/TDD users: 1-800-842-9710).
