      Manage a Condition | DiabetiCare - Diabetes | Know Your Benefits


      Diabetes is a condition that requires ongoing management by you and your doctor. Most ConnectiCare health plans provide benefits for diabetes education. If you have any questions, please contact Member Services.

      Members living in Connecticut have coverage for the following:

      1. A minimum of 10 hours of initial training for a member who is initially diagnosed with diabetes for the care and management of diabetes;
      2. A minimum of four hours for medically necessary training and education as a result of the development of new techniques and treatment for diabetes;
      3. Up to two hours per calendar year for counseling in nutrition and proper use of equipment and supplies for the treatment of diabetes.

      Members living in Massachusetts have coverage for the following:

      1. Outpatient self-management training and education, including medical nutrition therapy for the treatment of insulin-dependent diabetes, insulin-using diabetes, gestational diabetes and non-insulin using diabetes, when prescribed by a licensed health care professional and;
      2. The training must be provided by a certified, registered, or licensed health care professional trained in the care and management of diabetes.

      * Benefits may differ for members of self-funded groups; please consult your plan documents.
