Providers who may service ConnectiCare’s Medicare Advantage members with Choice Dual (HMO D-SNP) plans need to complete the Special Needs Plan Model of Care (SNP MOC) training no later than Sept. 15, 2022. The link to access the training course is ccisnpmoc.atlassystems.com.
The Centers for Medicare & Medicaid Services (CMS) requires providers to complete training for each dual-eligible special needs plan (D-SNP) they participate in.
Atlas will also send out instructions to our participating providers on how to complete the training online. The training takes only 15 minutes to complete, then follow the instructions to submit the attestation to receive the certificate of completion.
