Vision Benefits

Vision Exams

All of our plans include vision exams - There are two types of eye exams – routine and diagnostic.

  • Routine eye exam – A routine eye exam includes a vision check, screening for eye disease, and any updates to eyeglass or contact lens prescriptions. All our plans cover one routine eye exam per year, but you must visit an EyeMed® participating provider.

    Please visit EyeMed® or call toll-free 833-337-3134 (TTY: 711). The EyeMed® network includes independent providers and many familiar chains.

  • Diagnostic eye exam – A diagnostic exam tests to see if you have conditions that may need monitoring or treatment. Examples of those conditions are cataracts, glaucoma, detached retina, macular degeneration, cornea injuries, and dry eye. You’ll need to use the “Find a Doctor” directory to find in-network ophthalmologists (eye doctors).

Routine Eyewear

Some of our plans also include routine eye-wear allowance benefit. Allowances to help pay for routine eyewear are included in ConnectiCare Choice Plan 2 (HMO-POS), ConnectiCare Choice Plan 3 (HMO-POS), ConnectiCare Passage Plan 1 (HMO-POS), ConnectiCare Choice Dual (HMO-POS D-SNP), and ConnectiCare Flex Plan 3 (HMO-POS).

This vision allowance only covers routine eyewear dispensed by EyeMed® Insight network providers

Routine Eyewear includes one pair of glasses or contact lenses up to the allowed amount offered by your plan. The Evidence of Coverage document includes detailed information on coverage and copayments.

The allowance is applied at the time of your purchase – There are no reimbursement requests needed. No paperwork. No hassles.

Hearing Benefits


All of our plans include coverage for one routine hearing exam each year and for exams to diagnose and treat hearing and balance issues.

Some of our plans also include hearing aid allowance benefit. Allowances to help pay for hearing aids are included in ConnectiCare Passage Plan 1 (HMO-POS), ConnectiCare Choice Plan 2 (HMO-POS), and ConnectiCare Choice Dual (HMO-POS D-SNP).

You must obtain your hearing aids from a NationsHearing provider. Please contact NationsHearing by phone at 877-230-1549 (TTY: 711) 8 a.m. to 11 p.m., seven days a week or on the web nationshearing.com/connecticare to schedule an appointment. 

Your hearing aid benefit also includes:

  • Three follow-up visits within first year of initial fitting date.
  • 60-day trial period from date of fitting.
  • 60 batteries per year per aid (three-year supply).
  • Three-year manufacturer repair warranty.
  • One-time replacement coverage for lost, stolen or damaged hearing aid (deductible may apply per aid). 

The allowance is applied at the time of your purchase – there are no reimbursement requests needed. No paperwork. No hassles.

Nurse Hotline

A registered nurse is on call for 24 hour/7 day a week service. Members can speak one-on-one with a registered nurse, at any time. Nurses are trained in telephone triage and will provide clinical support for everyday health issues and questions if you have non-emergency health and medical questions. Your call is confidential.

There is no cost to you to call the nurse hotline: 877-489-0963 (TTY: 711)
