Vision Exams
All of our plans include vision exams - There are two types of eye exams – routine and diagnostic.
- Routine eye exam – A routine eye exam includes a vision check, screening for eye disease, and any updates to eyeglass or contact lens prescriptions. All our plans cover one routine eye exam per year, but you must visit an EyeMed® participating provider. Find EyeMed eye doctors at eyemedvisioncare.com and choose the “Insight” network. The EyeMed network includes independent providers and many familiar chains.
- Diagnostic eye exam – A diagnostic exam tests to see if you have conditions that may need monitoring or treatment. Examples of those conditions are cataracts, glaucoma, detached retina, macular degeneration, cornea injuries, and dry eye. You’ll need to use the “Find a Doctor” directory to find in-network ophthalmologists (eye doctors).
Routine Eyewear
Some of our plans also include routine eye-wear allowance benefit. Allowances to help pay for routine eyewear are included in Choice Plan 2 (HMO-POS), Choice Plan 3 (HMO-POS), Passage Plan 1 (HMO-POS), Choice Dual (HMO-POS D-SNP), Choice Dual Vista (HMO-POS D-SNP), and Flex Plan 3 (HMO-POS).
This vision allowance only covers routine eyewear dispensed by EyeMed® Insight network providers. Please visit the EyeMed website to find a participating provider in the EyeMed® Insight network, click “Find a Provider” and in the drop down choose “Insight Network.” or call 833-337-3134 (TTY: 711) 8 a.m. to 8 p.m., Monday through Friday to find a participating provider.
Routine Eyewear includes one pair of glasses or contact lenses up to the allowed amount offered by your plan. The Evidence of Coverage document includes detailed information on coverage and copayments.
